Dog Adoption Application

If you are interested in adopting one of our dogs or cats, please fill out the form below and then click the submit button. The form will be sent to us through email. You can also print the form, complete it, and mail it to Stray Rescue of St. Louis, 2320 Pine Street, St. Louis, MO 63103. If you don't feel comfortable sending some of the information over the internet, we will get the rest of the information when we call you.                                                   


Please Note: The adoption fees are as follows:

  • for puppies 6 months of age or younger is $150
  • for dogs older than 6 months is $100
  • all cats and kittens of all ages are $100

Fees are non-refundable. Fees help defray the cost of spay/neuter, vet care, food, etc. Our adoption fee includes the spay/neuter, micro-chipping and current vaccinations of the adopted pet, as well as a year of FREE training at the Pine Street shelter!  Post-adoption veterinary care is the financial responsibility of the new guardian.  

Please be advised that completing an application does not guarantee adoption. Our process includes a fully completed application with references and pet history, interview, home visit and agreement of terms. There are some cases where as the applicant could be turned down. Also, there could already be multiple applications on the specific companion animal. YOU WILL WANT TO CHECK THE STATUS OF THE PET YOU'RE INTERESTED IN ADOPTING TO MAKE SURE THEY DON'T ALREADY HAVE AN ADOPTION PENDING.

Transitional Behaviors

Please be aware: dogs can display a variety of behaviors during the transition time (can be anywhere from days, to weeks, to months) until they are comfortable in your home. Dog disagreements are expected and typical behavior when a new dog is brought into a home where a dog is already living, as well as some other common behaviors listed below. Which, if any, of these behaviors would you be COMPLETELY UNWILLING to work with during the transitional period?

Terms of Adoption

All pets must be spayed or neutered prior to adoption.

Dogs require much of your time, love, and attention. The costs of owning a dog include food, vaccinations, heartworm testing and preventative, licensing, possible boarding and emergency veterinary care. It may also require the expense of a trainer. Please consider these requirements when adopting a dog, and be sure you are willing to commit to a new family member who will be with you for the next 10-20 years.

I hereby affirm that I have answered the above questions completely and truthfully. I give my permission for Stray Rescue of Saint Louis to contact the landlord, veterinary and personal references I have provided, and I give my permission for these references to release any information they deem relevant to the adoption of a pet from Stray Rescue of Saint Louis, While meeting with animals I will not hold Stray Rescue, or any of its representatives, liable if an animal were to bite or scratch myself, or any member of my party.
