Adoption Application

If you are interested in adopting one of our dogs or cats, please fill out the form below and then click the submit button. The form will be sent to us through email. You can also print the form, complete it, and mail it to Stray Rescue of St. Louis,Â 1463 South 18th Street, St. Louis, MO 63104. If you don't feel comfortable sending some of the information over the internet, we will get the rest of the information when we call you.

Please Note: The adoption fee for dogs is $150.00 and $100.00 for cats, fees are non-refundable. Fees help defray the cost of spay/neuter, vet care, food, etc. Also please remember that just because you applied for a dog/cat does not mean that you automatically get the dog/cat. Our process includes a fully completed application with references and pet history, interview, home visit and agreement of terms. There are some cases where as the applicant could be turned down. Remember too, there could already be multiple applications on the specific companion animal. YOU WILL ALSO WANT TO CHECK THE STATUS OF THE PET YOU'RE INTERESTED IN ADOPTING TO MAKE SURE THEY DON'T ALREADY HAVE AN ADOPTION PENDING.

Our adoption fee includes the spay/neutering/microchipping and current vaccinations of the adopted pet. Any additional booster vaccinations needed after dog/cat is adopted are the financial responsibility of the new guardian.

Terms of Adoption

All pets must be spayed or neutered prior to adoption.

If, for any reason or at any time, I am unable to care for my adopted pet, I agree to return my adopted pet to Stray Rescue of St. Louis; I further agree never to abandon my adopted pet or deposit my adopted pet at an animal shelter or research facility; however, before returning my adopted pet to Stray Rescue, I agree to exhaust all options available to me including, but not limited to, obedience training, a behaviorist, crate training or veterinary advice to solve any physical or behavioral problem that may occur with my adopted pet or between me and my adopted pet.

I agree that by accepting my pet for adoption, I am accepting guardianship of the animal. I understand this means that Stray Rescue of St. Louis is not responsible for any damage to person or property that may be caused by my adopted pet after the date of the adoption and I accept full responsibility for my adopted pet from the date of the adoption forward.
